	Search Criteria (Multiple Criteria can be entered)	Directions for search
Pharmacy Type		Leave on Select Value if you want all Pharmacy Types.
License		Enter the complete License number including the dash. i.e. (83-458)
Corporation / Company Owner Name		Enter any part of the name for which you wish to search. A search for 'os' will result in 'Osborn' as well as 'Hoskins'
Business (DBA) Name		Works like the License Name Search.
City		Works like the License Name Search.
State		Enter the 2 letter state abbreviation.
Include Supplier		Check this box if you want pharmacies that are Drug Suppliers.
Include Sterile Compounding		Check this box if you want pharmacies that are Sterile Compounded.
Include Training		Check this box if you want pharmacies that have Training Areas.
Include Unused Drug Dispensing		Check this box if you want charitable pharmacies that can dispense Unused Drugs.
Include LTC Kit		Check this box if you want pharmacies that have LTC Emergency Kit.
Current Only		Check this box if you only want pharmacies with a current license.
